Information Collected for Health Risk Assessment

When you enter health related data on the “Health Risk Assessment” or similar applications on this Site, we will need to know your age, gender, weight, height, ideal body weight, and various basic information on health history, diet, exercise activity, social connections, and risk behaviors.  You agree to provide this information voluntarily.  Non-specific information that you provide (including, but not limited to positive health changes such as weight loss and other health related factors), may be used as part of aggregate studies by ichangebillings to track the progress of programs in the areas.  Such aggregated information will be used to identify best practices that produce positive health results in the area.  No specific names, telephone numbers, emails, or other individually specific information we be utilized as part of any aggregated studies or data analysis. We do not collect personally identifiable information about you except when you provide such information to us on a voluntary basis.  It is ichangebillings’ policy to comply with all applicable laws and regulations relating to its use of the information collected on this Site to the extent such laws apply to it and its operations. We will not sell, share, or rent this information to others in ways different from what is disclosed in this Privacy Policy
